As your professional Care Manager, I will provide expert advice, assistance and support as you face difficult challenges and decisions.

Comprehensive evaluation of individual’s current level of function & current living arrangements.
Develop personalized plan of care to maximize individual’s health and safety.
Referrals to appropriate facilities and agencies, as needed.
Ongoing monitoring to ensure individual’s safety and quality care.
Assistance to family members in implementing planning.
Liaison services to family members living out of town.
Assist with transitioning to assisted living or nursing home.
Ongoing education to caregivers regarding client’s conditions & needs.
